Obviously not straight. So, either Equius stands oddly just cause, or, he may have scoliosis.
Now, what’s scoliosis?
Scoliosis is a spinal deformity, that causes your spine or neck to be curved.
So, here is Equius’ original talksprite. It appears that he is pushing his chest outwards.
In “Equius: Seek the Highb100d” he is hunched.
Now, to answer your question, yes, scoliosis is painful, it depends on how large the curve is of course, and if the vertebrae are twisted, if nerves are being pinched, etc.
What causes most of the pain, is muscle tension, from not being about to support your back. So, the weaker your muscles are, the more it hurts.
You probably haven’t heard of scoliosis, but it is actually quite common.
About 3 in 100 people have scoliosis, though some, it is very minor, for others, like myself it causes constant pain from strained muscles.
However, if your muscles are STRONG, then the pain is much less apparent.
So, that is my headcanon. Equius have scoliosis in his upper back.
